If you've been told your hormone levels are “normal” but you still don't feel like yourself, you're not alone.
Women come to my practice at many different stages of life and with many different symptoms. Sometimes it's a woman in her 20s or 30s struggling with PCOS, difficulty getting pregnant, bloating, breast tenderness, heavy periods, fatigue, low libido, anxiety, or difficult PMS.
Other times, it's a woman in her late 30s or 40s who suddenly notices that her body doesn't respond the way it used to. She may be experiencing:
Midsection weight gain
Poor sleep
Increased anxiety or feeling “wired but tired”
Heavier or changing periods
Breast tenderness or worsening PMS
Low libido
Brain fog or poor memory
Fatigue and low energy
Difficulty building or maintaining muscle
Depression or mood changes
Less ability to handle stress
And later, symptoms may become much more obvious: hot flashes, night sweats, vaginal dryness, joint discomfort, terrible new anxiety, disrupted sleep, low energy, brain fog, changes in body composition, or simply the feeling that “I don't feel like myself anymore.”
Hormones absolutely matter. But one of the things I've learned caring for women is that hormones rarely tell the entire story.
In many ways, perimenopause can reveal what has been developing quietly underneath the surface for years.
Inflammation. Insulin resistance. Nutrient depletion. Gut dysfunction. An overactivated nervous system. Years spent running in fight-or-flight. Poor sleep. Sluggish elimination and hormone metabolism. A body that has been compensating beautifully—until suddenly it can't compensate quite as well anymore.
I often describe this as losing some of our physiologic reserve.
And typically, the stronger our foundations are, the less loudly our hormones have to scream to get our attention.
That's why my approach to women's hormone health isn't simply about prescribing estrogen, progesterone, or testosterone. At Flourish Functional Health, we look at the hormonal transition itself—and the health of the systems underneath it.
Why Hormone Symptoms Are Often More Complex Than “Low Hormones”
Hormones don't operate independently.
They communicate with and are influenced by nearly every major system in the body, including:
Blood sugar and insulin regulation
Thyroid function
Stress physiology
Gut health and the microbiome
Liver function and hormone metabolism
Nutrient status
Sleep and circadian rhythm
The nervous system
Environmental exposures
Emotional and mental well-being
This helps explain why two women of the same age with similar hormone levels can feel completely different.
One may move through perimenopause with relatively few symptoms. Another may experience profound sleep disruption, anxiety, weight gain, fatigue, heavy periods, brain fog, and a dramatically reduced ability to tolerate stress.
Hormone levels are part of the picture. The terrain those hormones are operating within matters too.
My Philosophy: Hormones Matter, But So Does the Terrain
Bioidentical hormone therapy can be life-changing for the right woman. Hormone therapy remains the most effective treatment for menopausal vasomotor symptoms and genitourinary syndrome of menopause and can prevent bone loss and fracture.
There are times when no supplement, herb, diet, or lifestyle intervention can fully replace what a declining hormone should be doing in the body. I prescribe bioidentical hormones when they are clinically appropriate, and I have seen hormone therapy make a tremendous difference in women's quality of life.
But I also don't believe every woman needs every hormone.
There is no universal “optimal hormone protocol.” If there were, my job would be easy.
A woman in her late 30s with poor sleep, heavy periods, and worsening PMS may need something very different from a 52-year-old experiencing hot flashes, vaginal dryness, joint pain, and sleep disruption.
So we look at your symptoms, health history, stage of life, risk factors, laboratory findings when appropriate, and—most importantly—how you respond.
I also prefer to make changes thoughtfully. Whenever possible, I add one layer of support at a time and reassess.
Sometimes we even begin with a smaller dose than a commonly used starting dose because some women are remarkably sensitive to hormonal shifts. Their bodies may need time to adapt.
The goal isn't to get every woman onto estrogen, progesterone, and testosterone.
The goal is to determine what your body actually needs.
Progesterone: Often Where the Story Begins
One misconception about hormone health is that everything suddenly changes at menopause.
For many women, changes begin much earlier.
In my clinical experience, I frequently see women begin experiencing symptoms consistent with changing progesterone patterns in their 30s and early 40s, while they're still having menstrual cycles.
As ovulation becomes less consistent, progesterone exposure can change, and women may notice:
Worsening PMS
Breast tenderness
Heavy periods
Anxiety
Poor sleep
Mood changes
Irregular cycles
Feeling less resilient to stress
For some women, progesterone is the first hormone support we consider.
That doesn't mean every symptomatic woman in her 30s needs progesterone. It means we look at the pattern rather than waiting until a woman reaches menopause before taking her symptoms seriously.
Estrogen: Needs Often Change as Menopause Approaches
As women move further through perimenopause and toward menopause, estrogen can fluctuate considerably and eventually declines.
This is when symptoms such as these may become more prominent:
Hot flashes
Night sweats
Vaginal dryness and urinary symptoms
Brain fog
Poor memory
Joint discomfort
Sleep disruption
Mood changes
Changes in body composition
Loss of bone density
For the appropriate patient, estrogen therapy can become an important part of the treatment plan.
But even here, there isn't one perfect delivery method for every woman.
Some women do beautifully with an estradiol patch or gel. Others may need a different formulation or may use a carefully compounded preparation when an individualized formulation is clinically appropriate.
I don't believe in forcing every woman into the same hormone protocol simply because it works well for someone else.
Testosterone: Important for Women, But Not Necessary for Everyone
Testosterone is important in female physiology too—but not every woman needs testosterone therapy.
For most women, androgen levels change gradually with age. Some women maintain adequate levels well into menopause, while others may have lower levels earlier.
I also sometimes see lower androgen levels in women during periods of significant or prolonged physiologic stress. Stress, sleep, illness, medications, nutrition, ovarian function, and adrenal androgen production can all be pieces of a much larger picture.
That's why I don't automatically assume that fatigue, low libido, loss of motivation, or difficulty maintaining muscle means a woman needs testosterone.
Symptoms matter, and testing can help us understand the broader picture and safely monitor therapy when it is used.
When testosterone is appropriate, there are several ways it may be prescribed in clinical practice, including:
Topical testosterone gels
Compounded testosterone creams
Injectable testosterone
Every women is different in how she responds to testosterone, some do great with gels, other with creams, others with injections.
Whatever the formulation, more is not necessarily better. I prefer to introduce hormone support thoughtfully and reassess. For some women, that means beginning very conservatively rather than trying to rapidly reach a predetermined number. The goal is symptom improvement without negative side effects.
Sometimes testosterone becomes an important piece of the puzzle.
Sometimes it doesn't need to be added at all.
And testosterone is not a guarantee to fix low libido, often it can help, but libido has so many more variables at play, particulary the health of the relationship and how significant stress levels are.
What About DHEA?
DHEA is produced primarily by the adrenal glands and serves as a precursor to other sex hormones.
DHEA-S levels naturally peak in early adulthood and gradually decline with age. Because much of a woman's DHEA comes from the adrenal glands, DHEA-S can be one window into adrenal androgen production and the broader stress picture—but not as a stand-alone test of “adrenal reserve.”
After prolonged periods of physiologic stress, I may see women with lower DHEA-S alongside fatigue, decreased resilience, or other symptoms. In select patients, we may discuss whether carefully replacing DHEA makes sense.
I think about this within the larger balance between stress, recovery, and rebuilding rather than simply trying to push a laboratory value higher.
As with testosterone, I consider symptoms, laboratory findings, age, medications, health history, and the rest of the hormonal picture before deciding whether it belongs in someone's plan.
Building the Foundation Beneath Your Hormones
This is the piece of hormone care I wish more women understood.
Whether or not you ultimately use hormone therapy, there are foundational systems that almost every woman benefits from supporting.
In my practice, we pay particular attention to:
Stress physiology and nervous system regulation
Thyroid function
Blood sugar and insulin sensitivity
Whole-food nutrition
Gut health and the microbiome
Liver function and hormone metabolism
Regular bowel function and elimination
Sleep and circadian rhythm
Movement, strength, and muscle
Nutrient sufficiency
Why?
Because hormones have to operate inside the environment your body gives them.
Addressing chronic stress, thyroid dysfunction, blood sugar instability, inflammation, nutrient depletion, gut health, constipation, and hormone metabolism can help strengthen the physiologic environment in which your hormones are produced, metabolized, cleared, and received by your tissues.
Sometimes strengthening these foundations changes how a woman feels dramatically.
Sometimes she still needs hormone therapy—and now we have created a stronger foundation underneath it.
These approaches aren't competing with each other. They belong together.
Your Gut Is Part of Your Hormone Story
Gut health may seem unrelated to estrogen or progesterone, but the relationship is more connected than many women realize.
The intestinal microbiome participates in estrogen metabolism, while bowel regularity influences the elimination of hormone metabolites.
This is why I pay attention to symptoms such as:
Constipation
Bloating
Food intolerances
Altered bowel habits
Chronic digestive symptoms
Supporting adequate fiber, whole foods, regular bowel movements, microbiome health, and digestive function may therefore become part of a broader hormone plan.
The goal isn't a trendy “detox.” It's supporting the normal pathways your body already uses to metabolize and eliminate hormones.
Blood Sugar, Insulin Resistance, and Midsection Weight Gain
One of the most frustrating experiences for women in midlife is watching their body composition change despite doing many of the same things that worked previously.
Being told to simply “eat less and exercise more” doesn't capture the complexity of what's happening.
Insulin resistance and blood sugar dysregulation can influence:
Midsection weight gain
Cravings
Energy
Inflammation
PCOS
Hormonal signaling
Body composition
At the same time, changing ovarian hormones can influence metabolism and body composition.
So instead of simply cutting calories lower and lower, I often look at the larger metabolic picture: food quality, protein, fiber, muscle mass, movement, sleep, stress physiology, and insulin sensitivity.
PCOS Is More Than an Ovary Problem
PCOS is another excellent example of why hormone care requires a whole-body approach.
Women with PCOS may come to me because of:
Irregular periods
Difficulty getting pregnant
Acne
Excess facial or body hair
Hair thinning
Weight gain
Difficulty losing weight
Fatigue
But PCOS frequently involves more than reproductive hormones.
Insulin resistance, inflammation, sleep, stress physiology, nutrition, and other metabolic factors may all be part of the picture.
This is why treating PCOS isn't simply about making a period regular or lowering an androgen level. We want to understand what is driving the pattern in that particular woman.
Stress, Cortisol, and the Nervous System
This is one of the most important—and often overlooked—parts of women's hormone health.
Many women reach their 40s after spending years caring for children, building careers, managing households, caring for aging parents, navigating relationships, sleeping too little, and carrying an enormous mental load.
The body can compensate for a long time.
Then perimenopause arrives, ovarian hormones become less predictable, sleep becomes more fragile, and suddenly the same amount of stress feels completely different.
Women tell me:
“I used to be able to handle everything. Now the smallest thing overwhelms me.”
That experience is real.
Supporting the nervous system, sleep, recovery, boundaries, movement, nutrition, and stress resilience isn't an optional “wellness extra” in my approach.
It's part of hormone care.
Supporting Healthy Hormone Metabolism
Having enough hormones is only part of the equation. Your body also has to metabolize and eliminate them appropriately.
The liver plays a central role in hormone metabolism, while the gut and bowel influence what happens to hormone metabolites before they leave the body.
This is why a hormone plan may also involve:
Supporting adequate protein and nutrient intake
Eating fiber- and phytonutrient-rich whole foods
Supporting regular bowel movements
Addressing significant gut dysfunction
Reducing excessive inflammatory burden
Supporting healthy liver function
Improving blood sugar regulation
Reducing unnecessary environmental exposures
Again, the goal isn't to “detox” your hormones aggressively.
It's to support the physiology your body already uses to process them.
Bioidentical Hormone Therapy: Personalized to You
When hormone therapy is appropriate, I prescribe bioidentical hormones—hormones that are chemically identical to hormones produced by the human body.
Depending on the individual, treatment may include:
Oral Progesterone
Oral micronized progesterone is commonly used when progesterone support is indicated.
Estradiol Patches or Gels
Transdermal estradiol provides systemic estrogen through the skin and is an excellent option for many women.
Compounded Topical Hormones
Some women may benefit from customized compounded preparations when commercially available products don't provide an appropriate dose, formulation, or ingredient profile.
Vaginal Estrogen
Local vaginal estrogen can be particularly helpful for genitourinary symptoms of menopause, including vaginal dryness and certain urinary symptoms.
Testosterone
When clinically appropriate, testosterone may be delivered through carefully dosed topical preparations such as gels or creams or even injections.
The important point isn't which formulation is universally “best.”
It's which evidence-informed approach makes sense for you.
Hormone Therapy Is Never One-Size-Fits-All
I don't expect your hormone needs at 38 to necessarily be your hormone needs at 48—or 58.
Hormone care is a process.
We may begin by strengthening foundational physiology and supporting progesterone. Later, estrogen may become appropriate. Testosterone may or may not ever be necessary. Doses and delivery methods may change as your body changes.
And throughout that process, we continue paying attention to the things underneath the hormones: thyroid function, stress physiology, blood sugar, gut health, nutrition, sleep, movement, inflammation, and healthy hormone metabolism.
I prefer to listen, support what needs support, and reassess rather than prescribe everything at once.
